Job Summary

We are seeking a skilled Nurse Practitioner to join our team. The successful candidate will provide high-quality patient care and education in an ambulatory care setting.

Key Responsibilities
  • Assess patients' health needs and develop individualized care plans.
  • Provide education and counseling to patients and their families.
  • Collaborate with healthcare teams to ensure seamless patient care.
  • Maintain accurate clinical records and documentation.
Requirements
  • Bachelor's Degree in Nursing and Master's Degree in Family Nurse Practitioner.
  • Minimum two years of experience as a Registered Nurse in an ambulatory care setting.
  • New York State Registered Licensed Registered Professional Nurse and Nurse Practitioner certification.
  • Basic Cardiac Life Support (BCLS) and Advanced Cardiac Life Support (ACLS) certifications.
  • Family Nurse Practitioner certification from the American Nurses Credentialing Center.
  • Excellent written and verbal communication skills.
