Job Summary

We are seeking a skilled Registered Nurse to join our transitional care team. As a member of our team, you will play a vital role in providing holistic nursing care to residents, supporting their physical, emotional, and social well-being.

Responsibilities
  • Deliver high-quality nursing care to residents in accordance with established standards and policies
  • Conduct thorough assessments and develop individualized care plans to meet the unique needs of each resident
  • Collaborate with interdisciplinary teams to ensure seamless transitions of care
  • Provide education and support to residents and their families on health-related topics and disease management
Requirements
  • Associate's Degree in Nursing (ADN) or Bachelor's Degree in Nursing (BSN)
  • Current Washington State Registered Nurse License
  • National Provider BLS Certification from the American Heart Association within 30 days of hire
