The Position

As a Nurse Practitioner or Physician Assistant with Matrix Medical Network, you will conduct adult/geriatric assessments, medical history reviews, diagnoses, treatments, health education, physician referrals, case management referrals, follow-up, and clear documentation according to Matrix guidelines and protocols.

You will work collaboratively with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as necessary.

This role involves providing services in various settings, including home visits, skilled facility visits, and telehealth visits.

Responsibilities
  • Conduct comprehensive health assessments, review medical histories, diagnose, and treat patients as needed
  • Work closely with healthcare teams to ensure seamless patient care
  • Provide educational support to patients and families on managing chronic conditions
  • Collaborate with primary care physicians on patient care and follow-up
  • Travel to patients' locations for home visits
Requirements
  • Master's degree required or equivalent experience and satisfactory completion of NP licensure
  • Current RN and NP or PA licensure in the state of practice, including prescription authority or ability to obtain prescriptive authority
  • Board certification by NCCPA, AANP, ANCC, or AACCN in a Matrix-approved specialty
  • At least 1 year of experience as a Nurse Practitioner or Physician Assistant
  • Strong computer skills and familiarity with EMR software
  • Possess excellent verbal and written communication skills
  • Ability to travel, possess a valid state driver's license, and drive a car with adequate automobile insurance coverage
Compensation

$95.00 per completed assessment
