We are seeking a highly skilled Healthcare Professional to join our team as a Care Coordination Specialist. In this role, you will work collaboratively with physicians and multidisciplinary team members to develop comprehensive care plans for patients.
Responsibilities- Assess patient needs and develop individualized care coordination plans
- Collaborate with healthcare teams to ensure seamless transitions of care
- Facilitate communication between patients, families, and healthcare providers
- Identify and resolve barriers to effective care delivery
- Minimum 3 years of experience in nursing or a related field
- Licensed Registered Nurse (Pennsylvania) - RN_State of Pennsylvania
- Bachelor's Degree-Nursing (Preferred)
- Estimated Salary: $83,500 - $114,300 per year
