Job Summary:

This role will involve conducting health assessments, recording medical history, and providing comprehensive care to patients with complex wounds. The successful candidate will possess strong communication skills, a passion for delivering high-quality care, and the ability to work effectively within a collaborative team.

Responsibilities:

  • Conduct thorough health assessments, including taking complete medical histories and performing physical examinations as needed
  • Develop and implement individualized treatment plans, incorporating advanced wound care techniques and evidence-based practices
  • Collaborate with multidisciplinary teams to ensure seamless care coordination and optimal patient outcomes
  • Document patient progress, communicate effectively with healthcare providers, and maintain accurate records
  • Participate in ongoing education and training to enhance clinical knowledge and stay up-to-date on best practices in wound care
  • Maintain a clean driving record and valid driver's license, with regular travel required to patient residences and skilled nursing facilities

Requirements:

  • Board-certified Nurse Practitioner or Physician Assistant (APRN) in the state of Texas
  • Advanced Wound Care Certification (AWCC) or equivalent preferred
  • Minimum 2 years of experience in wound care, with prior debridement experience highly valued
  • Strong communication and interpersonal skills, with ability to build rapport with patients, families, and healthcare professionals
  • Ability to work independently, prioritize tasks, and manage time effectively in a fast-paced environment
