Job Description
  • The LPN will assume responsibility for the delivery of care for patients in a long-term care/assisted living facility.
  • The nurse will demonstrate ability to make clinical judgments in an effective and efficient manner with supervision, as well as critical thinking and performance ability in the coordination of patient care.
  • Additionally, this individual will perform according to nursing standards and be accountable in managing patient care and assisting others in the management of patient care.
Essential Duties and Responsibilities:
  • Utilize the nursing process to assess, plan, implement, and evaluate patient care in a long-term care/assisted living facility.
  • Perform focused interviews to identify specific needs.
  • Assess signs and symptoms indicating physiologic and psychological changes in the patient's condition.
  • Collect, analyze, and interpret data and information from health care members and document actual and/or potential nursing diagnoses.
  • Document the patient's plan of care using identified nursing diagnoses, expected patient outcomes, and selected nursing interventions.
  • Identify and prioritize actions based upon patient care requirements.
  • Monitor consistency between patient plan of care and actions of medical staff.
  • Perform interventions according to identified priorities, plan of care, and the facility policies and patient outcome standards.
  • Provide a safe, comfortable, and therapeutic patient environment.
  • Initiate and evaluate patient and/or family education.
  • Revises the plan of care according to evaluation, changes in medical plan of care, and effective/ineffective nursing interventions.
  • Document patient's response to nursing and other interventions.
  • Use clinical judgment in evaluation activities to meet patient care needs of an assigned floor/unit including establishing priorities.
  • Monitor safety and effectiveness of the environment and equipment.
  • Participate in activities to facilitate good interpersonal communication with the chosen facility.
  • Support professional practice by observing the following: Uses knowledge and practices in accordance with 1) the Nurse Practice Act; 2) Standards of Nursing Practice; 3) Standards of Patient Care; 4) Joint Commission, state and local laws; 5) Patient's Bill of Rights; and 6) Amergis Healthcare, nursing, and client hospital policies.
  • Perform other duties as assigned/necessary.
Minimum Requirements:
  • Current Licensed Practical Nurse License for the state in which the nurse practices.
  • Ability to effectively elicit/provide information to and from appropriate individuals via strong spoken, written, listening, and nonverbal communication skills.
  • Current Health Certificate (if applicable).
  • Current PPD or Chest X-Ray.
  • One year prior professional nursing experience preferred.
  • Current BLS card.
  • Successful completion of screening tests for the clinical specialty area(s) in which the LPN practices with a score of 80% or greater.
  • Successful completion of Background Screening Process.

Amergis offers a competitive salary ranging from $45,000 to $65,000 per year, depending on experience.
