Banner Health is seeking a skilled Registered Nurse (RN) to join our team as a Nurse Navigator in the Progressive Care Unit (PCU) at our Phoenix, Arizona location.
Job Summary:
This is a full-time, salaried position that requires excellent interpersonal and communication skills. The successful candidate will work independently to provide comprehensive care coordination, supporting a holistic and coordinated approach across the continuum of care.
Key Responsibilities:
- Assess patient's plan of care and develop, implement, monitor, and document the utilization of resources and progress of the patient through their care.
- Evaluate medical necessity and appropriateness of care, optimizing patient outcomes.
- Assist patients and families through diagnostic services, treatment, and care, coordinating disease care using an interdisciplinary holistic approach.
- Ensure patients and families understand diagnostic processes, care strategy, and recommended actions, providing appropriate information in coordination with physicians and healthcare providers.
- Support patients and families during difficult decision-making periods, assisting in coordination of end-of-life care and providing emotional support.
- Establish and promote a collaborative relationship with physicians, payers, and other members of the healthcare team, collecting and communicating pertinent, timely information to payers and others to fulfill utilization and regulatory requirements.
Requirements:
- Must possess knowledge as normally obtained through the completion of a Bachelor's degree in nursing or related field.
- Requires a current RN licensure in the state of practice, BLS certification, and a proficiency level typically achieved with 5 years of clinical experience.
- Excellent organizational skills, clinical knowledge regarding specialty care services, and care coordination of services, legal and financial aspects of diagnostic services and health services in specialty area.
- Effective communication and writing skills, good time management skills, and knowledge of word processing and database software applications.
