We are seeking an experienced Licensed Practical Nurse (LPN) to join our team at Capital Area Health Network. In this role, you will play a critical part in delivering high-quality patient care and education.

Job Summary

The LPN will be responsible for providing comprehensive nursing care to patients in our Wellness Center. This includes conducting health assessments, developing individualized care plans, and coordinating with the interdisciplinary team to ensure seamless delivery of services.

Key Responsibilities:

  • Triage patients who present with medical emergencies and determine their medical state, following up with medical providers as needed.
  • Coordinate day-to-day clinical patient flow to ensure patient safety and provide solutions to any challenges or obstacles within the clinical team.
  • Travel from site to site to provide direct patient care, triage, and other duties as assigned.
  • Conduct health/educational assessments and re-evaluations during intake and conduct all components of the initial triage process.
  • Act as a patient advocate and consult with physicians where necessary.
  • Assist with the coordination of resources with the interdisciplinary team.
  • Develop policies, procedures, and clinical protocols in collaboration with the team.
  • Participate in case conferences with the interdisciplinary team and provide supporting facts where necessary.
  • Maintain accurate and up-to-date patient files in accordance with HIPAA regulations.
  • Process routine, urgent, and Stat referrals.
  • Understand insurance processes and requirements.
  • Attend team meetings and provide clinical updates where necessary.

Requirements:

  • Must have a valid Virginia driver's license, satisfactory driving record, and transportation to travel to our various medical offices.
  • Must be licensed to practice in the state of Virginia with a minimum of three years of nursing experience.
  • Current CPR (Healthcare Provider) certification is required.
  • Previous experience as a Licensed Practice Nurse within a community health and/or public health setting is preferred.
  • Leadership skills and knowledge, skills, and critical thinking abilities are essential.
  • Computer literacy and previous experience with computerized charting are required.

Compensation and Benefits:

The estimated salary for this position is $65,000 - $75,000 per year, based on experience. We also offer a comprehensive benefits package, including health, dental, and vision insurance, retirement savings plan, and paid time off.
