Job Title: Early Intervention Service Coordinator

We are seeking a highly motivated and experienced Early Intervention Service Coordinator to join our team at Fayette County Board of Developmental Disabilities.

Job Summary:

The Early Intervention Service Coordinator will work closely with families of children ages birth to three years of age who have a developmental delay or a diagnosed medical condition. The successful candidate will provide advocacy, support, and coordination of services to ensure the best possible outcomes for these children and their families.

Key Responsibilities:
  • Advise families of their rights and responsibilities in the Early Intervention process
  • Assist parents in developing their role as advocates for their child
  • Maintain confidentiality of records and information
  • Organize and manage ongoing written documents, file management, data entry, and correspondence
  • Collaborate with providers and school districts to plan the transition phase to preschool or other community programs
  • Coordinate evaluation and assessments, and develop Individualized Family Service Plans to ensure ongoing services are provided to meet the family and child's needs
Requirements:
  • Bachelor's degree in early childhood education, social work, or a related field
  • 1 year of experience in early intervention services in DD or a related field (preferred)
  • Minimum of an associate degree required for certification
  • Certification from the Ohio Department of Developmental Disabilities
  • CPR and First Aid certification
  • Ability to pass criminal background checks and obtain a valid Ohio driver's license with a clean driving record
