Social Worker

We are seeking a compassionate Social Worker to join our team at LHC Group. As a Social Worker, you will play a vital role in assisting patients and families in coping with the challenges of severe or long-term illness, and with difficulties in recovery and rehabilitation. Your expertise will be invaluable in assessing, diagnosing, and treating patients' mental and social conditions, counseling individuals and/or families, and updating case records.
About the RoleIn this role, you will instruct health care team members on community resources available to assist patients, plan for continuity of care with hospitals and community agencies, and assess and treat social and emotional factors related to patients' illnesses to determine their ability to cope with daily living problems. You will also assist patients and their significant others to understand, accept, follow, and implement medical recommendations.
Responsibilities- Assess patients' mental and social conditions and develop effective treatment plans.
- Counsel individuals and/or families to help them cope with the challenges of illness and treatment.
- Update case records to ensure accurate and up-to-date information.
- Collaborate with healthcare team members to provide comprehensive care.
- Develop and maintain relationships with community resources to support patient care.
To be successful in this role, you will need to have a Master's Degree from a school of Social Work accredited by the Council of Social Work Education, as well as at least one year of social work experience in a healthcare setting. You must also have current CPR certification and a valid driver's license.
What We OfferWe offer a competitive salary of $65,000 - $80,000 per year, depending on experience.
