Speech Language Pathologist - PRN

Vertis Therapy is seeking a detail-oriented and passionate PRN Speech Language Pathologist to join our team at our beautiful Assisted Living in Anderson, IN. As a therapist-owned and operated company, we focus on patient-centered care and provide a supportive environment for our clinicians to thrive.

Job Summary

The Speech Language Pathologist will evaluate and treat patients in accordance with physician orders, using therapeutic procedures to address deficits in speech, language, hearing, swallowing, and cognition. The successful candidate will participate as a member of the interdisciplinary team, providing support and information within the area of speech therapy.

Responsibilities
  • Conduct thorough functional assessments of patients, utilizing standardized procedures and documenting patient progress.
  • Develop and implement individualized treatment plans, containing achievable functional goals and revising plans as needed throughout the treatment process.
  • Record patient progress in the patient record, a minimum of every seven (7) days, and document the course of treatment in a clear and concise manner.
  • Orient patients to program services and treatment procedures, informing patients of any potential risks during procedures.
  • Work with nursing staff, patients, and families to maximize patient use of functional abilities and establish exercises and therapeutic procedures to be continued following discharge.
  • Assist in the referral process, maintaining current knowledge of community resources and participating in rehabilitation team conferences regarding patient progress, problems, or needs.
  • Attend and participate in other department/facility meetings, as required, and access continuing education opportunities to maintain clinical accuracy.
  • Participate in the quality improvement process, responding appropriately to results of medical record audits, patient/referral source satisfaction surveys, safety activities, and regulatory requirements.
  • Act in compliance with Vertis Therapy policies and procedures, Federal and Professional Organization regulatory and professional standards and guidelines, and promote the programs and services of Vertis Therapy through formal and informal interactions with the community.
Requirements
  • Master of Science degree in Speech/Language Pathology.
  • Current license/certification in the state of practice.
  • Prefer minimum one (1) year experience in geriatric rehabilitation.
  • Must meet the educational requirements for certification and be in the process of accumulating the supervised experience or meet the education and experience requirements for a Certificate of Clinical Competence in speech-language pathology granted by the American Speech-Language Hearing Association.
  • Excellent oral and written communication and interpersonal skills.
  • Adequate computer skills to complete required job duties and computerized documentation.
  • Demonstrated competency in applying principles, methods, materials, and equipment used in speech therapy.
