Salary: $30-$35 per hour

Job Description:

We are seeking an experienced Licensed Vocational Nurse (LVN) to join our team as a Home Health LVN/LPN. In this role, you will provide high-quality nursing care to patients in their homes, working closely with healthcare teams to develop and implement individualized care plans.

Responsibilities:

  • Provide skilled nursing visits according to treatment plans prescribed by physicians
  • Maintain accurate and complete medical records
  • Communicate effectively with patients, families, and healthcare teams

Requirements:

  • One year of clinical experience and one year of home health experience
  • Currently licensed LPN or LVN in the state of agency operation
  • Valid CPR certification
  • Licensed driver able to travel to all business locations
