Job Title: Clinical Nurse Specialist

We are seeking a skilled and compassionate Clinical Nurse Specialist to join our team at Fairfield County Hospice House Inc. As a key member of our interdisciplinary care team, you will play a vital role in providing high-quality patient care and supporting the emotional and social needs of our residents.

About the Role:
This is a per-diem and full-time opportunity for an experienced RN/LPN to work collaboratively with our multidisciplinary team in delivering comprehensive care to terminally ill residents. You will have the opportunity to make a meaningful impact on the lives of our patients and their families.

Key Responsibilities:
- Provide direct patient care, assessing and implementing individualized care plans to optimize pain and symptom management.
- Collaborate with healthcare providers, family members, and other support staff to ensure seamless care delivery.
- Supervise and support Certified Nursing Assistants (CNAs) in their daily tasks, promoting a culture of teamwork and professional growth.

Requirements:
- 1-3 years of relevant clinical experience, preferably in hospice or palliative care.
- Excellent communication and interpersonal skills, with the ability to build strong relationships with colleagues, patients, and families.
- Strong organizational and time management skills, with a focus on prioritizing tasks and meeting deadlines.

Compensation:
We offer a competitive salary range of $65,000 - $90,000 per year, commensurate with experience.
