About the Role
">- We are seeking a highly motivated and compassionate Homeless Services Advocate to join our team at PATH in San Jose, CA.
Job Summary:
">The Homeless Services Advocate will work directly with individuals experiencing homelessness, providing critical support and resources to help them achieve stable housing and improved well-being. This role is ideal for someone passionate about making a difference in their community and committed to empowering vulnerable populations.
">Responsibilities:
">- Conduct intakes and individualized needs assessments for clients, collaborating to develop personalized service plans that address housing goals, income maintenance, and other essential objectives.
- Provide referrals, linkages, information, and support to valuable resources, helping clients attain their service plan goals.
- Evaluate strengths and challenges to address short-term and long-term objectives, conducting 90-day reviews with clients after initial intake.
- Maintain accurate and up-to-date documentation, adhering to program standards and PATH policies.
- Promote confidentiality of client files, ensuring HIPAA compliance and adherence to all applicable guidelines.
- BUILD STRONG RELATIONSHIPS WITH CLIENTS AND PARTNERS TO ENHANCE EFFORTS TO ENGAGE AND RETAIN INDIVIDUALS IN THEIR HOUSING GOALS.
Requirements:
">- A minimum of 2 years of experience in healthcare, case management, homeless services, or working with vulnerable populations.
- Analytical skills, attention to detail, collaboration, communication, creativity, critical thinking, and ethics.
- Driving is an essential function of this position; candidates must have reliable transportation, a valid driver's license, proof of insurance, and vehicle ownership.
- Ability to work flexible hours, including evenings and weekends, as needed.
