About the Role

We are seeking an experienced Registered Nurse (RN) to join our team in providing compassionate care to patients in their homes. The ideal candidate will have a strong background in hospital or prior home health experience, with excellent communication and interpersonal skills.

Responsibilities

  • ">
  • Conduct thorough assessments to identify patient needs, develop personalized care plans, and revise as necessary;">
  • Perform various medical procedures, including cardiovascular, pulmonary, neurological, GI, skin, intake and output, and pain assessments;">
  • Administer medications, maintain accurate records, and ensure compliance with Medicare guidelines;">
  • Provide education and counseling to patients and families on disease management, self-care techniques, and prevention strategies;">
  • Collaborate with multidisciplinary teams to coordinate patient care, address concerns, and resolve issues;">
  • Maintain accurate documentation, complete required paperwork, and adhere to Covenant Health policies and procedures;">
  • Communicate effectively with patients, families, physicians, and other healthcare professionals to ensure continuity of care;">
  • Supervise LPNs and Home Health Aides, provide training, and evaluate performance;">
  • Foster a culture of respect, empathy, and compassion within the organization;">

Requirements

  • ">
  • Graduation from an accredited school of nursing;">
  • One year of professional experience in hospital or prior home health experience;">
  • Active State of Tennessee RN license and CPR certification;">
  • Valid Tennessee driver's license Class D and minimum insurance coverage;">
  • Demonstrated ability to work independently, prioritize tasks, and manage time effectively;">
  • Strong analytical, problem-solving, and decision-making skills;">
  • Excellent communication, interpersonal, and teamwork skills;">
