Physician Assistant Home Health Therapist

Job DescriptionWe are seeking a highly skilled Physical Therapist to join our fast-growing team in Hayward, CA and surrounding areas. As a leading provider of medical and non-medical home care services, we offer a unique opportunity for professionals who want to make a difference in the lives of patients.
The successful candidate will have a strong focus on providing patient-centered care, with a commitment to delivering high-quality treatment programs that meet the individual needs of each patient.
Key Responsibilities:
- Planning and administering prescribed skilled physical therapy treatment and training for patients suffering from various injuries, illnesses and functional disabilities.
- Developing/implementing a conditioning/rehabilitation program consistent with physician's Plan of Treatment and the overall goals of the patient/rehab team.
- Conferring with physician and clinical team members to obtain additional patient information and assist in developing, implementing and revising the therapy treatment program and Plan of Treatment.
The salary for this position is estimated at $120,000 per year, depending on experience, plus competitive per-visit rates and performance incentives. Requirements and QualificationsTo be considered for this role, candidates must have:
- An advanced degree of Doctor of Physical Therapy (DPT) from an institution accredited by the American Physical Therapy Association (APTA).
- A current and unrestricted license to practice Physical Therapy in the State of California.
- Current CPR/BLS certification.
- Possess up-to-date knowledge of treatment practices and physical therapy programs.
