This vital field role entails direct patient care and case management within the community, aiming to optimize healthcare outcomes. As an RN Case Manager, you will assess patient conditions, develop appropriate care plans, and maintain comprehensive medical records.
Key Responsibilities:
- Assess patient conditions and develop appropriate care plans.
- Maintain comprehensive medical records and report patient status to necessary parties.
- Act as the primary point of contact among patients, their families, and healthcare providers.
- Educate patients and families on health management and preventive measures.
- Coordinate with healthcare professionals to ensure continuous care and efficient case management.
Requirements:
- Active RN license in Idaho with no restrictions.
- At least 1 year supporting IN HOME patients.
- Proficient in case management software, excellent organizational and communication abilities.
