Nurse Care Coordinator

Geisinger Health Plan is seeking a dedicated Nurse Care Coordinator to join our team in Wilkes-Barre, Pennsylvania. As a key member of our case management team, you will play a vital role in optimizing the health status of patients and members.
About the RoleThis position offers a competitive salary of $93,350 per year, plus additional benefits and perks. As a Nurse Care Coordinator, your primary responsibility will be to assess, plan, implement, coordinate, monitor, and evaluate options and services to optimize patient or member health status.
You will work closely with healthcare teams, patients, and families to develop and implement individualized care plans that promote quality and efficiency in healthcare delivery. Your expertise will help us further refine our care model to maximize clinical, quality, and fiscal outcomes.
Responsibilities- Develop evidence-based treatment plans that are patient-centric, promoting quality and efficiency in healthcare delivery.
- Create systems of care that monitor progress and promote early intervention in acute care situations.
- Assist in designing, implementing, and evaluating advanced patient-centered care models.
- Evaluate healthcare, educational, and psychosocial needs of patients or members.
- Design individualized care plans and foster a team approach by working collaboratively with patients, families, primary care providers, and other healthcare team members.
- Continuously evaluate laboratory results, diagnostic tests, utilization patterns, and other metrics to monitor quality and efficiency results for assigned populations.
- Work to apply benefits and utilization management appropriately, serving as a resource to patients, members, and healthcare teams.
- Maintain required documentation for all case management activities.
- Collect data and utilize it to adjust treatment plans when necessary.
To be successful in this role, you must have a minimum of 3 years of RN work experience and be licensed as a Registered Nurse in Pennsylvania. A Bachelor's Degree in Nursing is preferred.
