Job Summary

We are seeking an experienced LPN to join our team as a Licensed Home Health Nurse Specialist. This role involves providing skilled nursing care to patients in their homes, working independently and collaboratively with other healthcare professionals.

Responsibilities
  • Collects and documents patient data, including assessments and observations, to inform care plans.
  • Implements current care plans at the direction of the RN, ensuring timely and effective delivery of services.
  • Provides skilled nursing care, treatments, and preventive procedures as assigned, demonstrating a commitment to patient safety and well-being.
  • Documents accurate and complete notes of patient interactions, treatments, and care delivered, contributing to the revision of care plans as needed.
  • Contributes to the evaluation of patient response to care plans through documentation and verbal communication with the interdisciplinary team.
  • Supports, educates, and empowers patients, families, and caregivers regarding nursing care needs and related problems.
  • Assists patients in learning self-care techniques, promoting independence and self-management.
  • Demonstrates respect for patient privacy and property, adhering to HIPAA guidelines.
  • Serves as a liaison between patients, families, and healthcare providers, facilitating seamless communication and coordination of care.
  • Administers medications and treatments according to established protocols and OH Board of Nursing regulations.
  • Participates in case conferences and QAPI meetings to discuss patient progress and identify opportunities for improvement.
Requirements
  • Graduate of an accredited school of practical nursing with current State of OH LPN license.
  • Minimum 2 years of recent nursing experience in acute, long-term care, or home care settings, with at least 1 year in home care preferred.
  • Strong patient teaching skills, written and verbal communication skills, and ability to work independently.
  • Current BLS certification, valid OH Driver's License, and proof of insurance.
  • Basic computer skills and proficiency in electronic health records systems.
Working Conditions

This position requires flexibility in meeting changing workflow demands, frequent interruptions, and emergency situations.
