Job Summary

The Physician Assistant/Nurse Practitioner is a vital member of our healthcare team, responsible for assisting physicians in conducting hospital rounds and seeing patients independently for office visits.

This role requires a strong ability to work collaboratively with physicians and other healthcare providers to provide high-quality patient care across the continuum of care.

Essential Duties and Responsibilities
  1. Works in collaboration with physicians and other healthcare providers to provide healthcare services to assigned patients.
  2. Performs hospital rounds of new consults and follow-up patients, and assists in the discharge of patients.
  3. Obtains patient medical, physical, and psycho-social history, and performs comprehensive health assessments, including physical exams, based on age and history.
  4. Conducts non-invasive preventive screening procedures based on age and history.
  5. Identifies medical and health risks and needs, and updates and records changes in health status in the electronic medical record.
  6. Formulates the appropriate differential diagnosis based on history, physical examination, and clinical findings.
  7. Orders and interprets appropriate diagnostic tests and laboratory studies within protocol guidelines.
  8. Develops and provides relevant patient education plans.
  9. Prescribes and administers appropriate pharmacologic and non-pharmacologic therapies.
  10. Participates in assisting and performing common minor urological procedures.
  11. Makes appropriate referrals to other healthcare professionals and community agencies.
  12. Determines the effectiveness of the plan of care through documentation of client care outcomes.
  13. Reassesses and modifies the treatment plan as necessary to achieve medical and health goals.
  14. Completes all documentation in the electronic medical record in a timely fashion, and codes patient visits accurately and appropriately.
  15. Participates in quality assurance review on a periodic basis, including systematic review of records and treatment plans.
  16. Treats patients and their families with respect and dignity.
  17. Consults other departments, as needed and as appropriate, to provide for an interdisciplinary approach to patient care.
Education and Experience

Graduate of an accredited Physician Assistant/Nurse Practitioner program; NYS licensed.

1-5 years related experience; or equivalent combination of education and experience.

Qualifications and Requirements

Demonstrates an ability to utilize advanced health assessment and decision-making skills.

Utilizes initiative, strives to maintain a steady level of productivity, is a self-starter, compatible and supportive, and a team player.

Maintains a good working relationship and effective communication both within the department and with other departments for the benefit of the patient.

Demonstrates an ability to be flexible, organized, and function under stressful situations.

Maintains patient confidentiality at all times.

Represents the organization in a positive and professional manner.

Attends outside clinical updates as required, and attends annual review and department in-services as scheduled.

Complies with all regulatory requirements and standards.

Actively participates in performance improvement and continuous quality improvement activities.
