Role Summary
We are seeking a skilled Nurse to join our team as an At Home Hospice Nurse. In this role, you will be responsible for planning, implementing, documenting, and coordinating patient care services to meet the unique needs of our patients and their families.

Key Responsibilities
- Provide high-quality, person-centered care to patients in their homes
- Collaborate with interdisciplinary teams to develop and implement patient care plans
- Conduct thorough assessments and document patient information accurately
- Communicate effectively with patients, families, and healthcare professionals

Requirements
To succeed in this role, you must have:

  • A current New York State Registered Nurse License/Permit
  • At least two years of experience in nursing, including medical-surgical, oncological, palliative, and community home health/hospice settings
  • Preferred qualifications include one year of community hospice or home health experience and a NYS driver's license

Location and Hours
This position is located in Westchester and requires a regular schedule of 9am - 5pm.
