Our dedicated care teams work together to help patients rehabilitate, recover, and regain their independence.

Your Role: As a Clinical Assessment Advisor, you will be responsible for completing and overseeing the development of patient treatment plans, performing ongoing evaluations, and coordinating with healthcare professionals to ensure optimal clinical outcomes. You will also work closely with physicians and referral sources to obtain relevant clinical information and ensure seamless care coordination.

Key Responsibilities:

  • Conduct initial and ongoing assessments of patient needs within your scope of practice
  • Develop and implement individualized care plans that promote health maintenance, prevention, and safety
  • Collaborate with multidisciplinary teams to manage care plans and achieve desired outcomes
  • Communicate effectively with patients, families, and healthcare providers to ensure continuity of care
  • Participate in special projects and perform other duties as assigned

Requirements:

  • Bachelor of Science degree in Nursing (BSN) preferred
  • At least 2 years of clinical home care experience, with a minimum of 1 year in a clinical management role
  • Current CPR certification
  • Excellent verbal and written communication skills
  • Knowledge of state and federal home health agency regulations and compliance standards
  • Ability to read, write, and speak fluent English

  • A compensation range of $69,800 - $96,200 per year, based on demonstrated job-related skills, knowledge, experience, education, certifications, and performance
