We are seeking a skilled Care Manager RN to join our team at Pacific Medical Centers. As a key member of our care management team, you will play a vital role in coordinating patient care across various settings, ensuring cost-effective and appropriate utilization of healthcare resources.
Key Responsibilities- Coordinate patient care plans, collaborating with physicians, nurses, and other healthcare professionals to ensure seamless transitions and optimal outcomes.
- Conduct thorough assessments of patient needs, identifying opportunities for care improvement and developing strategies to address complex health issues.
- Develop and implement effective care plans, incorporating evidence-based practices and patient-centered care principles.
- Monitor patient progress, making adjustments to care plans as needed to ensure optimal outcomes.
- Collaborate with interdisciplinary teams to ensure timely and effective communication, reducing healthcare disparities and improving patient satisfaction.
- Washington Registered Nurse License
- 5 years of clinical experience
- Utilization review and case management experience
- Bachelor's Degree in Nursing
- Certified Case Manager (CCM) certification
