We are seeking a skilled Registered Nurse - RN to join our team at Crestwood Center in Milford, NH. As a Registered Nurse, you will provide high-quality patient care and work collaboratively with our interdisciplinary team to deliver exceptional healthcare services.

Key Responsibilities:
  • Perform comprehensive nursing assessments and develop effective plans of care in collaboration with patients, families, and healthcare teams.
  • Delegate patient care responsibilities to staff and supervise them to ensure tasks and interventions are implemented.
  • Communicate patient information with assigned staff and between shifts to ensure seamless care delivery.
  • Establish realistic, measurable short and long-term patient goals with interventions in place to minimize rehospitalizations and continued evaluation of outcomes.
