Youth Transition Specialist

We are seeking a highly skilled Youth Transition Specialist to join our team at Kids Central, Inc.
Job SummaryThe primary responsibility of this role is to ensure that young adults transitioning out of foster care have the necessary knowledge, skills, and resources for successful independence. This includes providing education on life skills, employment opportunities, and building a network of supportive adults.
This position requires excellent communication and interpersonal skills, with the ability to work independently and collaboratively with a team. The ideal candidate will possess a strong passion for working with teens and young adults, and be able to assess risk and make sound judgments.
Key Responsibilities- Serves as primary case manager for youth aged 18-23 years
- Serves as secondary case manager for youth aged 17 years
- Schedules transition meetings and creates personalized plans for success
- Conducts quarterly home visits with youth in licensed foster care
- Ensures youth receive life skills training and tracks their progress
- Assists with educational workshops and events
- Supports various youth functions, including the Youth Advisory Council and IL Graduation Celebration
- Prepares and processes necessary paperwork for youth participating in the Road to Independence Program
To be considered for this role, candidates must possess a Master's degree in Human Services or a related field. A Bachelor's degree with a waiver may also be accepted. Relevant experience in child welfare is preferred, but not required. Certification in Child Welfare is a plus.
Candidates must also be able to maintain a clear driving record, valid Florida driver's license, and proof of automobile insurance limits. Travel up to 65% is required for this role.

The estimated salary for this role is between $55,000 - $70,000 annually, based on location and experience. This figure is subject to change and may not reflect the actual salary offered.
