Job Description

We are seeking an experienced RN Case Management professional to join our team in Hillsboro, Oregon. As a case management nurse, you will play a critical role in assessing, planning, facilitating, and coordinating care for patients. Your expertise will be instrumental in promoting patient safety, quality of care, and cost-effective outcomes.

You will work closely with the healthcare team to ensure seamless communication and effective utilization of services and resources. Your ability to communicate effectively with patients, families, and other healthcare professionals will be essential in achieving optimal results.

Required Skills and Qualifications
  • At least 1 year of recent acute care experience in specialty
  • Active RN license
