Care Management Coordinator

As a Care Management Coordinator, you will provide direct service, coordination of care, and linkage to services and advocacy, as needed, to individual clients and/or families. Your goal is to assist clients with their overall life management and recovery.
Schedule:
Mon-Fri, First Shift
You will provide needs assessment, referral, education, advocacy, and follow-up services to clients for issues concerning mental health, medical, substance abuse, vocational, and living skills. You will collaborate with other providers, agencies, and individuals in the client's network of care. You will also provide referral, advocacy, and education to the community regarding issues of chemical dependency or mental health. You will monitor medication regimen and document findings. You will assist clients in developing and strengthening their support network to accomplish goals and objectives. You will provide assistance with living arrangements and income maintenance to ensure safe and affordable housing. You will develop and implement case management plans and document services consistent with Thrive standards. You will produce, complete, and maintain billing forms, logs, data collection forms, case management documentation, and client records on time and in accordance with Thrive standards and state and federal regulations. You will schedule and maintain client hours sufficient to achieve individual/program billing targets. You will accompany clients to community appointments, provide advocacy as needed. You will review email account daily for new messages. You will have a Bachelor's Degree in the Human Service field or an equivalent combination of education and experience. You will have a minimum of 2 years of experience in the Human Service field working with clients with disabilities. You will have experience or training in related state and federal assistance programs. You will have basic computer literacy skills. You will have a valid driver's license and automobile insurance that meets Rhode Island minimum standards, or the ability to obtain necessary transportation to perform the responsibilities/tasks of the job. You will have Case Management/Community Support Professional Certification or be working toward it.

As a Care Management Coordinator, you will report directly to the Program Manager and will be supervised according to Thrive Behavioral Health Supervision Policy.
