The RoleThis is a full-time opportunity based in our Wicker Park office. As a Primary Care Physician or Advanced Practitioner, you'll play a vital role in managing patient panels with diverse needs, conducting a mix of acute, chronic, and well visits, and collaborating with in-office teammates via daily huddles. You'll also have opportunities to perform in-office procedures and utilize your clinical training to deliver exceptional care.
Responsibilities- Manage patient panels with varied needs and conduct a mix of acute, chronic, and well visits
- Treat patients in-office or testing centers, as well as conduct occasional tele-health visits
- Participate in continuous learning during weekly Clinical Rounds and other modalities
- Collaborate with in-office teammates through daily huddles and virtual clinical teams
- Utilize specific clinical training and perform in-office procedures
To be successful in this role, you must have:
- Completed an accredited FNP or PA program with national certification
- Prior experience practicing as an Advanced Practitioner in an outpatient primary care setting, seeing patients of all ages (0+)
- State licensure in IL, obtained by your start date
