About the Role

We are seeking a dedicated and compassionate Care Navigator Specialist to join our team at Society of St. Vincent de Paul South Pinellas. This is a full-time position that requires a high level of organization, communication, and problem-solving skills.

Job Description

The Care Navigator Specialist will be responsible for providing personalized support to clients, helping them navigate the system and achieve their goals. This includes registering new clients, setting attainable goals, monitoring progress, and providing assistance with various needs such as housing, apartment searching, veteran assistance, and referrals.

The successful candidate will have excellent communication and listening skills, be able to work in a fast-paced environment, and possess a strong commitment to helping others. They will also be responsible for maintaining accurate records, following up with clients, and reporting progress to management.

Responsibilities
  • Register all new clients between 2:30 p.m. and 6:00 p.m.
  • Set attainable goals and monitor progress of clients.
  • Provide assistance with various needs such as housing, apartment searching, veteran assistance, and referrals.
  • Maintain accurate records and reports.
  • Follow up with clients and report progress to management.
Requirements

To be successful in this role, you will need:

  • A high school diploma or equivalent.
  • Basic computer skills.
  • Excellent communication and listening skills.
  • Ability to work in a fast-paced environment.
  • Strong commitment to helping others.
