Embedded Cardiologist for Primary Health Team

Corewell HealthWest's Primary Health department is seeking a skilled cardiologist to join their team as an embedded physician in primary care. This role is designed to enhance coordination and integration between primary and secondary care in a value-based care model. The ideal candidate will have a strong background in cardiology and excellent communication skills to work effectively with the primary care team.
Key Responsibilities
- Assess and monitor the medical needs of patients/significant others.
- Provide quality medical services to patients in a cost-effective, appropriate manner and ensure coverage of case load at all times.
- Contribute to the development of policies and procedures related to medical practice and quality patient care.
- Oversee Physician Assistants and Nurse Practitioners and others providing delegated services to patients.
- Evaluate patients'/significant others'/staff educational needs and develop and implement appropriate teaching strategies or delegate to appropriate resources.
- Document in a manner consistent with regulatory standards.
- Communicate with other members of the health care team to ensure continuity of care and coordination of services. Act as a patient/family advocate.
- Participate in ongoing process improvement activities through the development/application of clinical paths, process improvement and other related activities.

- Required Bachelor's Degree
- LIC-Physician (MD) - STATE_MI State of Michigan Upon Hire required Or
- LIC-Osteopathic Physician (DO) - STATE_MI State of Michigan Upon Hire required
- CRT-DEA Registration - STATE_MI State of Michigan Upon Hire required
- CRT-Basic Life Support (BLS) - AHA American Heart Association 120 Days required Or
- CRT-Basic Life Support (BLS) - ARC American Red Cross 120 Days required
- CRT-Adv Cardiovascular Life Support (ACLS) - AHA American Heart Association If required by specialty or stress test performed 120 Days required
