About the Role:
We are seeking a highly skilled Care Manager to join our team at Meridian Illinois. As a Care Manager, you will play a critical role in developing and implementing personalized care plans for our members, focusing on mental and behavioral health needs.
Key Responsibilities:
- Evaluate member needs and develop comprehensive care plans to ensure high-quality, cost-effective healthcare outcomes.
- Collaborate with healthcare providers, specialists, and community resources to identify and address member needs.
- Provide education and support to members and their families on mental health and substance use disorder topics.
- Monitor care plans and member outcomes, making recommendations for adjustments as needed.

Requirements:
- Master's degree in Behavioral Health or Social Work, or a related field.
- 2-4 years of experience in care management or a related field.
- Licensed Behavioral Health Professional or RN, depending on state contract requirements.
