Job Summary

We are currently seeking a highly skilled and dedicated Geriatric Care Specialist to join our team at Bonsai Rehab. This is a full-time opportunity for a Physical Therapist to work in the Home Care setting, primarily serving the geriatric population in the Summerville, SC area.

About the Role

This position offers flexibility in terms of schedule and hours, allowing the ideal candidate to determine their patient load based on their needs. We welcome both experienced and new graduate Physical Therapists, with all caseloads billed under Medicare Part-B.

Responsibilities
  • Deliver high-quality physical therapy services to patients in their homes, ensuring optimal care and outcomes.
  • Maintain accurate records and documentation of patient progress, adhering to regulatory requirements.
  • Collaborate with other healthcare professionals to develop and implement comprehensive treatment plans.
  • Provide education and support to patients and their families, promoting independence and self-management.
  • Stay up-to-date with industry developments and best practices, contributing to ongoing professional growth and development.
Requirements
  • A state license as a Physical Therapist is mandatory.
  • New graduates are encouraged to apply, along with experienced professionals.
  • The successful candidate must be willing to work full-time hours and have a minimum of 10 hours of direct patient care per week.
  • Availability of three weekdays is required, with some flexibility in scheduling.
Compensation

We offer a competitive salary range of $80,000 - $110,000 annually, depending on experience and qualifications.
