Compass Health Network is seeking a dedicated and compassionate Community Support Specialist to join our team

The Community Support Specialist plays a vital role in providing integrated, holistic community support services to children and their families. This position requires a strong understanding of the System of Care philosophy and a person-centered, strengths-based approach to service delivery.

Clinical Service Provision
• Collaborate with clients and families to identify strengths, needs, and goals, and develop individualized service plans to support their movement towards improved functioning and achieving treatment goals.
• Work closely with parents to engage and participate in treatment with their child, ensuring improved clinical outcomes.
• Provide services to children and their families using a holistic, person-centered approach, incorporating coaching on the Eight Dimensions of Wellness and addressing each domain of wellness, including physical and mental health.
• Support clients and families in completing goals identified on Wellness Plans, reviewing the WP with them as required, and reassessing their needs and service choices available.
• Assist families in accessing and arranging services and resources to achieve their goals, and maintain contact with clients and families when they are hospitalized, participating in discharge planning and communicating with all involved providers to develop after-care plans.
• Provide educational information and skill-building to clients and families on areas of concern, such as daily living skills, coping skills, education, symptom management, and communication/relationships, with a focus on improving functioning.
• Develop and implement crisis intervention plans with families and provide crisis intervention services as needed, during and after normal working hours.
• Assist clients and families in developing natural support systems, including work with family members, school staff, legal guardians, etc.
• Participate in and coordinate with the child and family interagency team members on an ongoing basis to advocate for the Mental Health needs of children and families.
• Complete documentation collaboratively with clients and families and complete all required documentation within the required timeframe, according to agency policy.
• Meet service provision expectations per client needs and agency guidelines, providing services in the home with clients and families based on clinical need, including evenings and weekends.
• Other duties as assigned.

Integrated Health Team
• Communicate actively with other members of the integrated team, including psychiatrists, therapists, primary care physicians, specialists, nurses, dentists, NCMs, etc., regarding all areas of client wellness, including client adherence to medications, side effects, and efficacy of medications.

• Seek and include input from Integrated Health Team providers on client Wellness Plans
• Make referrals to additional services/providers as needed for client's Integrated Health Team
• Function as a liaison to others involved in client's treatment, including family, school, Children's Division, Juvenile Office, guardians, etc.
• Seek culturally competent services and service providers to meet the unique cultural needs of each agency client and family
• Driving is an essential function of the position
