Full Time Primary Care Provider

We are seeking a highly skilled Full Time Primary Care Provider - Geriatric Nurse Practitioner to join our team at Parker Jewish Institute for Health Care and Rehabilitation, conveniently located on the Queens-Nassau County border in New Hyde Park, New York.
This is an excellent opportunity for a dedicated and compassionate Nurse Practitioner to provide high-quality primary medical care to our patients in their homes, making a meaningful impact on their lives and health outcomes.
Responsibilities:- Examine patients and order appropriate diagnostic workups to ensure comprehensive care
- Assess members with acute change in condition in a timely manner to prevent complications
- Maintain complete and accurate medical records to inform treatment decisions
- Consult with other members of the interdisciplinary team to create effective care plans tailored to each patient's needs
- Educate patients and their families on disease management, medication adherence, and lifestyle modifications to promote healthy behaviors
- Provide medical management to optimize patient outcomes and quality of life
To be successful in this role, you will need:
- Licensure and current registration to practice as a Nurse Practitioner in New York State
- Strong communication and follow-up skills to build trust with patients and families
- Knowledge of Microsoft Office, including Excel, and experience with data entry, documentation, and report generation
- Prior experience working with geriatric populations and home care settings is highly desirable
- Excellent medical documentation skills to ensure accuracy and compliance
- Fully vaccinated against COVID-19 to protect yourself and others
