Job Summary

We are seeking a dedicated and compassionate Home Health Aide to join our team at 365 Health Services. As a Home Health Aide, you will provide high-quality care to our clients in the comfort of their own homes.

Key Responsibilities
  • Assist patients with personal care activities, including hygiene, mobility, and elimination, as prescribed in the plan of care.
  • Practice Universal Infection Control Precautions in all contacts with patients.
  • Perform or assist patients/families with home maintenance activities, including light housekeeping and meal preparation, as prescribed in the care plan.
  • Use safe transfer, ambulation, normal range of motion, and positioning techniques.
  • Respect the autonomy and right to confidentiality of all patients and families.
  • Document care provided and response of consumer/family, submitting documentation in a timely manner.
  • Perform within the limits of preparation and experience.
  • Ensure compliance with local, state, and federal laws, Medicare regulations, and established agency policies and procedures.
  • Meet or exceed 365 Health Services' established service standards.
  • Conduct all business activities in a professional and ethical manner.
  • Maintain regular attendance.
  • Adhere to 365 Health Services' Code of Conduct.
  • Interact with all staff in a positive fashion, supporting 365 Health Services' mission and vision.
