Job Summary

Under the direction of the Office Manager and Medical Director, this Registered Nurse will provide patient care by conducting assessments, administering treatment and medication, performing diagnostic tests, and arranging patient follow-up. The successful candidate will conduct telephone triage according to established protocols, record medical history, and complete documentation in the medical record. They will also coordinate patient care, acting as a liaison with other organizations, and promote and restore patient health by completing the nursing process.

Requirements
  • Current State of Michigan License (unrestricted) as a Registered Nurse (RN)
  • Bachelor of Science in Nursing (BSN) preferred
  • Current Basic Life Support (BLS/CPR) required
  • Previous outpatient experience preferred
Essential Functions and Responsibilities
  • Conduct telephone triage according to established clinical protocols, educating patients and guiding them to appropriate care
  • Maintain an active nursing appointment schedule for assisting providers in the management of patients with acute and chronic conditions
  • Identify patient care requirements by completing the nursing process
  • Provide condition-specific patient education and anticipatory guidance under the direction of providers
  • Assist care manager and provider in follow-up management of post-hospitalized or post-emergency room visits
  • Screen patients for care opportunities and collaborate with treating provider to identify referrals to care management
  • Communicate and coordinate with care team on clinical issues related to patient care
  • Administer medication and treatment according to clinical protocols and provider oversight
  • Maintain clinical documentation in the electronic medical record (EMR) and complete forms associated with patient care
  • Establish a compassionate environment by providing patients with emotional support and comfort
  • Protect patients and employees by adhering to infection prevention and control standards
  • Maintain and protect patient confidentiality
