Nurse Practitioner

At Sunrise Senior Living, we are seeking a skilled and compassionate Nurse Practitioner to join our team as a Resident Care Specialist. As a key member of our care team, you will play a vital role in delivering high-quality care to our residents and promoting their overall well-being.
Responsibilities:- Conduct regular wellness visits for all residents to identify on-going needs and services.
- Collaborate with the Resident Care Director to develop and implement individualized care plans.
- Monitor and document resident vital signs, weights, and medication profiles.
- Provide clinical support and assistance to community team members as needed.
- Participate in Sunrise Quality Assurance Meetings and Clinical Meetings to ensure compliance with regulatory requirements and promote quality care.
- Graduate of an approved college or school of nursing.
- Current state license as a professional Licensed Practical Nurse/Licensed Vocational Nurse (LPN/LVN).
- Minimum of one year experience in home health, assisted living, hospital, or long-term care environment.
- Demonstrated knowledge of nursing practices, techniques, and methods applied to health and wellness resident care.
- Ability to handle multiple priorities and possess excellent communication and interpersonal skills.
