Clinical Care Coordinator

As a valued member of the Johns Hopkins HealthCare team, you will play a critical role in ensuring the highest quality patient care. The Clinical Care Coordinator is responsible for implementing the plan of care for specific patients, beginning on a pre-admission basis and following through placement and into follow-up/after care. This position requires innovative skills for continuous improvement in patient outcomes and cost effectiveness. The Clinical Care Coordinator collaborates with the patient, family, Interdisciplinary Care Team members, and external healthcare providers to assure safe, timely, and efficient care. Key responsibilities include initiating the plan of care, monitoring its implementation, and facilitating mid-course care changes. What you will need:
- BSN required
- Minimum of five years of relevant clinical experience
- Prior case management experience highly desirable
Salary Range: $39.94 - $61.91 per hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility.
