Job Summary:

We are seeking a skilled Nurse Practitioner to join our team on a PRN/Part Time basis. As a Nurse Practitioner, you will conduct home risk assessments, medical history reviews, and provide health education to patients in their homes. You will work collaboratively with physicians, case managers, and other healthcare professionals to ensure comprehensive care.

Responsibilities:

• Conduct home risk assessments, medical history reviews, and provide health education to patients in their homes.
• Work collaboratively with physicians, case managers, and other healthcare professionals to ensure comprehensive care.
• Develop and implement individualized care plans to address patient needs and goals.
• Provide education and support to patients and their families to promote healthy behaviors and disease management.
• Collaborate with other healthcare professionals to ensure seamless care transitions and coordination.

Requirements:

• Master's Degree in Nursing required.
• Current RN and NP licensure in state of practice to include prescription authority or the ability to obtain prescriptive authority.
• Board certified by the AANP, ANCC or the AACCN in a Matrix approved specialty.
• Have a current BLS, ACLS or CPR certification.
• Have a Minimum of 1 year experience as a Nurse Practitioner.
• Have strong computer skills and familiarity with Employee health/medical record software.
• Possess excellent verbal and written communication skills with patient, clients and colleagues.

Travel Requirements:

• Have the ability to travel, a valid state driver license and able to drive a car, proof of adequate automobile insurance coverage for the state of residence.
• Travel required to meet patients where they live.

$95.00 or $100.00 based on county per completed Assessment.
