Care Coordinator for Older Adults

About the Role:
We are seeking a highly skilled Registered Nurse (RN) to join our team at Haven Behavioral Hospital of Philadelphia. As a Care Coordinator for Older Adults, you will play a critical role in providing exceptional patient care and coordination services.
Job Summary:
The successful candidate will provide direct patient care under the supervision of the DON, oversee the provision of care during assigned shifts, serve as a resource to direct care staff, and support the DON with hiring, orientation, staffing, staff education, performance improvement, and other activities as assigned.
Responsibilities:
- Provide direct patient care, including assessment, planning, implementation, and evaluation
- Coordinate care with interdisciplinary teams to ensure comprehensive treatment plans
- Supervise and mentor direct care staff to promote high-quality patient care
- Support the DON with personnel management, staff development, and performance improvement initiatives
- Collaborate with other departments to ensure seamless operations and excellent patient outcomes
Requirements:
- Graduate of an accredited RN program
- Current licensure as a Registered Nurse in Pennsylvania
- Ability to work effectively in a fast-paced environment with minimal supervision
- Excellent communication and interpersonal skills
- Ability to maintain confidentiality and handle sensitive information
Benefits:
