Nurse Practitioner or Physician Assistant, HouseCalls Opportunity

At Optum, we're revolutionizing healthcare by bringing care to patients in the comfort of their own homes. As a Nurse Practitioner or Physician Assistant on our HouseCalls team, you'll play a vital role in delivering high-quality, patient-centered care.
Key Responsibilities:- Conduct comprehensive in-home assessments, including evaluations of past medical history, medication reconciliation, and vital signs.
- Perform evidence-based practice screenings, including point-of-care testing as needed.
- Identify diagnoses and develop care management plans to ensure optimal patient outcomes.
- Communicate findings with patients' primary care physicians to inform care gaps.
- Respond to urgent and emergent situations, providing timely interventions as needed.
- Educate patients on disease processes, medication management, and compliance strategies.
- Active NP or PA licensure, with the ability to obtain national certification in the state of assignment.
- Reliable transportation to complete home visits.
- Ability to complete physical requirements of the job, including lifting a 30-pound bag and navigating various dwelling conditions.
- 1+ years of clinical experience in a related setting.
- Experience in gerontology, cardiology, internal medicine, or endocrinology.
- Fluency in Spanish, Cantonese, Korean, Vietnamese, Polish, or other languages.
