Salary Information

The estimated salary for this role is $40,000 with a $25,000 student loan repayment or sign-on bonus for individuals who have not previously participated in this program.

About the Role

This important preventive care helps identify and reduce health risks for patients, in addition to coordinating appropriate follow-up care to improve health and well-being. As a team member of our HouseCalls product, we provide annual clinical assessments to patients in the comfort of their homes.

Job Responsibilities
  • Conduct comprehensive in-home assessments, including evaluations of past medical history, medication reconciliation, vital signs, and comprehensive physical exams.
  • Perform evidence-based practice screenings, including point-of-care testing as necessary.
  • Identify diagnoses to inform care management and active medical management in furtherance of treatment.
  • Formulate a list of current and past medical conditions using clinical knowledge and judgment.
  • Communicate assessment findings to inform members' primary care physicians of potential gaps in care.
  • Educate members on disease processes, medication, and compliance.
Requirements
  • An active NP license or ability to obtain by start date, with unencumbered licensure.
  • Active ANCC or AANP national certification in Family, Geriatrics, Adult, Adult-Gerontology Primary Care, or Adult-Gerontology Acute Care certification, or ability to obtain national certification and/or NP license in state of assignment by start date.
  • Ability to complete the physical requirements of the job, including lifting a 30-pound bag and navigating stairs and varying dwelling conditions.
  • Access to reliable transportation to travel up to 85% within licensed states for home visit assessments.

To be eligible for the full benefit of the student loan repayments, employees must remain in an Advanced Practice Clinician or Physician role within Optum for 36 months.
