Job Summary

We are seeking a highly skilled Licensed Practical Nurse (LPN) to join our Hospital at Home team in Springfield. As a key member of our operations team, you will play a vital role in providing exceptional patient care and supporting our clinical team.

Key Responsibilities
  • Support hospital-based operations and logistics for the Hospital at Home program
  • Provide patient care under the supervision of the HaH Admitting MD/APP
  • Coordinate and facilitate patient screening and program enrollment
  • Maintain patient rights and confidentiality of patient information
  • Perform HaH assessments as directed by the HaH Admitting MD/APP
  • Coordinate all non-clinical activities based on the HaH Admission Checklist
  • Work cooperatively with hospital staff to ensure a great patient experience
Requirements
  • High school diploma or GED equivalent
  • Graduate of accredited nursing program
  • Current LPN license within hiring state
  • Excellent communication skills and attention to detail
  • Ability to self-organize and prioritize work in a fast-paced environment
Preferred Qualifications
  • 1+ years' experience with patient access and delivery systems
  • Prior healthcare clinical operations experience
