Job Summary:

Matrix Medical Network is seeking a skilled Nurse Practitioner to join our team as a Home Risk Assessment Specialist. In this role, you will conduct comprehensive health assessments for older adults and at-risk individuals in their homes, providing critical insights into their overall health and well-being.

Key Responsibilities:

  • Conduct Adult/Geriatric Assessments, medical history, diagnosis, and treatment, health education, physician referrals, case management referrals, follow-up, and clear documentation according to Matrix guidelines and protocols.
  • Work collaboratively with physicians, case managers, social workers, family members, key caregivers, and any appropriate ancillary medical personnel as needed.
  • Collaborate with Primary Care Physicians (PCPs) on patient education and provide follow-up.
  • Conduct Home Visits, Skilled Facility Visits, and Telehealth Visits.

Requirements:

  • Nurse Practitioner licensure and certification.
  • Strong computer skills and familiarity with Employee Health/Medical Record software.
  • Ability to travel, valid state driver's license, and adequate automobile insurance coverage.
