Job Description:
  • Key Responsibilities:
  • Serve as a liaison between clients, staff, and community resources to ensure comprehensive support services.

  • Develop and implement effective case management strategies to address client needs and goals.

  • Maintain accurate and detailed records of client interactions and progress.

  • Foster strong relationships with community partners to expand service offerings and improve client outcomes.

Required Skills and Qualifications:
  • Education:
  • Bachelor's degree in Social Work or related field.

  • Experience:
  • Minimum 3 years of experience in social work or a related field.

  • Skills:
  • Excellent communication and interpersonal skills.

  • Able to work independently and as part of a team.

  • Strong problem-solving and critical thinking skills.
