Heritage Park Care Center is seeking a dedicated and compassionate Social Services Assistant to join our team. As a Social Services Assistant, you will play a vital role in ensuring the medically related social and emotional needs of our patients are met.

 

Responsibilities:
  • Assist with the social services program to meet the medically related social and emotional needs of patients.
  • Document appropriately and timely, according to State law.
  • Support patients and families through education, financial planning assistance, liaison with community agencies, etc.
  • Exhibit excellent customer service and a positive attitude towards patients.
  • Assist in the evacuation of patients.
  • Demonstrate dependable, regular attendance.
  • Concentrate and use reasoning skills and good judgment.
  • Communicate and function productively on an interdisciplinary team.
  • Perform various tasks as assigned by the Social Worker(s).

 

Requirements:
  • High school graduate or equivalent.
  • Prior healthcare experience preferred.
