We are seeking a skilled Licensed Practical Nurse to join our Ambulatory Care team at NYC Health + Hospitals.

About the Role

This is a dynamic and challenging opportunity to provide high-quality patient care in an office practice setting. As an Ambulatory Care LPN, you will work collaboratively with a multidisciplinary team to deliver professional nursing care to clients and their families.

Job Description

The successful candidate will have excellent communication and interpersonal skills, as well as the ability to establish successful relationships with patients, coworkers, and other healthcare professionals. Key responsibilities include:

  • Ensuring compliance with National Patient Safety Goals (NPSG)
  • Conducting initial evaluations and developing individual care plans initiated by providers based on identified needs and treatment goals
  • Administering vaccines and medications as ordered by PCPs and directed by RNs
  • Assisting with diagnostic tests and procedures according to policy
  • Recognizing and responding to emergency situations effectively

You will also be responsible for motivating, listening, coaching, and interacting with chronic disease patients and their families to set and achieve self-management goals.

Requirements

To be successful in this role, you will need to possess the following qualifications:

  • Current licensure or authorization to practice as a Licensed Practical Nurse in New York State
  • Effective communication and interpersonal skills
