About the Role
We are seeking a per diem Registered Nurse Case Manager to join our team. This role will be responsible for assessing, planning, coordinating, monitoring, and evaluating options and services to meet an individual's health care needs across the continuum.
Key Responsibilities
- Assess patients' health care needs and develop comprehensive care plans
- Collaborate with interdisciplinary teams to coordinate care and services
- Monitor and evaluate the effectiveness of care plans and make adjustments as needed
- Communicate effectively with patients, families, and healthcare providers to ensure seamless care transitions
Requirements
To be successful in this role, you will need:
- A graduate degree in nursing from an accredited program
- At least one year of experience in case management or a related field
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
