About the Role

We are seeking a compassionate and skilled Hospice Care Specialist to join our team at Agape Care Group. This role is ideal for individuals who are passionate about providing exceptional care to patients and their families during their end-of-life journey.

Job Description

As a Hospice Care Specialist, you will be responsible for conducting psychosocial evaluations and ongoing counseling of patients and families. You will work closely with our interdisciplinary team to develop and implement personalized plans of care that address the unique needs of each patient. Your primary focus will be on providing emotional support and comfort to patients and their loved ones during this challenging time.

Key Responsibilities
  • Conduct thorough psychosocial assessments of patients and their families
  • Develop and implement individualized plans of care in collaboration with the healthcare team
  • Provide ongoing counseling and emotional support to patients and their families
  • Communicate effectively with patients, families, and healthcare providers to ensure seamless care coordination
Qualifications

To be successful in this role, you must have:

  • A Master's degree in Social Work from an accredited program
  • Current licensure as a social worker in the state where you reside
  • At least 2 years of experience in clinical social work, preferably in a healthcare or hospice setting
  • Excellent communication and interpersonal skills
  • A strong ability to empathize with patients and families

Salary

The estimated salary range for this position is $65,000 - $85,000 per year, depending on your level of experience and location.
