Home Health Nurse

We are seeking a skilled Home Health Nurse to join our team at Addus Homecare. As a Home Health Nurse, you will be responsible for providing high-quality nursing care to patients in their homes. Job Summary:
The Home Health Nurse will perform assessments and simple medical nursing procedures in client's homes, under the guidance of the RN Supervisor. The ideal candidate will have at least two (2) years of clinical practical nursing experience and be licensed as a vocational/practical nurse in the state of practice.
About the Role:
You will work closely with patients, families, and healthcare team members to coordinate patient care and maintain a high degree of confidentiality. You will also administer medications as instructed and document patient files accordingly.
Responsibilities:
- Perform selected nursing procedures, commensurate with experience and education within the scope of LPN/LVN practice
- Provide patient and family teaching according to plan outlined by the RN
- Observe, assess and record patient's physical and mental condition and reports any changes
- Administer medications as instructed and as ordered by the physician and instructed by the RN
- Document the patient's file accordingly
- Work with patient, family and health care team members effectively to coordinate patient care
- Maintain a high degree of confidentiality at all times due to access to sensitive information
- Follow all Medicare, Medicaid, and HIPAA regulations and requirements
Requirements & Competencies:
- Licensed as a vocational/practical nurse in the state of practice
- At least two (2) years clinical practical nursing experience
- Trach and Vent experience preferred
- Ability to make home visits and provide nursing care in the home environment
- Ability to complete necessary documentation appropriately, legibly and timely

Salary Estimate: $45.00 per hour, based on a flexible schedule and location in Mobile, AL 36604. The estimated annual salary is approximately $93,600.
